This has never happened to me. They're probably just outside the radar when you look and then walk in when you're not.

Sounds to me liek the extra 3 hydras are past your radar area when you are lookng at the 2 hydras and come into view after you move closer.
Like this:
......................your agrro ring.....Hydras' agrro ring
(---------------(-you-)----------(-2 hydras-)-)___(-3 more hydras-)
^................................................. .............^opposite edge of radar map
edge of radar map
As you can see, the 3 other hydras are just slightly out of your view range. They may be on a migrational path towards the other 2, and meet you on their own, the Henchies may aggro them, or they may be partied with the first 2 and coming running to help when you attack.

Lag has nothing to do with it. I've often done the same thing because the enemy was so clustered together they appeared as very few until I start the battle and they spread out into 5-6 or more. It can be frustrating but what I end up doing now is scanning that spot with the CTRL button down. It reveals the names of who's ahead and shows if its just 1 or 5 standing in the same place.

Well, the other thing i've noticed periodically (and this may just be something that happens to me, others can confirm) is that you get the nice "Jumping" more or less like what happens when you lag and get thrown back 30 feet or so ... i'll explain further, although my text-map drawing skills do nothing to rival Canadian Bacon's
First:
(you+party)------------------------------(patrol of critters)
Then you see:
(you+party)--------(one lonely mob, closer to you) ------- (remaining patrol of critters)
If you happen to try and attack that creature that's seemed to venture away from the group, you'll find he really is still in with the rest of them, and you'll end up aggroing the whole lot.
I don't think i'm explaining this well at all, hopefully someone else has had this same experience and can explain it better

A parital solution: Press the "Ctrl" button and look around to see what critters are nearby and how many are actually in the mob you're about to take on. I don't know how many times I've not done it and charged into a group that I thought were 3 or 4 enemies, but turned out to be 6 or 7!
And yeah, I've been ambushed by things that didn't show up on the radar or main screen while engaging a mob - no fun!
